This is an English translation provided for information purposes. In case of any discrepancy, the Portuguese version of this statement prevails.

Personal data statement – consent

For the purposes of Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (General Data Protection Regulation – GDPR), I hereby give my consent to the processing of my personal data listed in the registration form by the Associação Independente Karate-Do Wado Ryu (Independent Wado Ryu Karate-Do Association), tax number 503013510, with registered office at Rua Florentino Pereira Mota, n.º 12 – 2.º Dto., 2005-278 Santarém – Portugal.

This statement constitutes sufficient authorisation for AIKW to process my personal data in connection with registration with the club, Associação Independente Karate-Do Wado Ryu, in marketing activities, social media, advertising and the regional, national or international competitions it is affiliated with for the sports season concerned, provided that they fall within the scope of the club.

The personal data whose processing is authorised by this statement may not be used for any direct marketing or other commercial purposes, including profiling or any other automated decision-making, and may be subject to portability under Article 20 of the GDPR.

Your rights

I further declare, under Articles 13 to 22 of the GDPR, that I have been informed of the following rights regarding my personal data held in the said database:

  • To withdraw my consent to the processing of my personal data;
  • To object to the further processing of my personal data;
  • To request from the data controller access to my personal data, as well as its rectification or erasure, including the exercise of the “right to be forgotten”;
  • To lodge a complaint with the Portuguese Data Protection Authority (CNPD) or another supervisory authority for personal data protection;
  • To be informed, upon request, of the purposes of the processing, the categories of data involved, the identity of the recipients to whom it has been disclosed and the period for which my personal data will be kept;
  • To be informed of which personal data is being processed and of any available information on the source of that data, electronically, if not through this document;
  • To consult, access, rectify, update or delete my personal data provided when registering for tournaments, championships or other initiatives, by sending an e-mail to aikwado@gmail.com.
